Reviewing Parent Submissions
How to review and approve submissions completed by parents
The Review Process
After a parent submits their declaration, staff must review it before approval. The review page displays all submitted information in organised sections.
Review Page Sections
The review page shows the following information sections:
1. Child Information
- Name, known as, date of birth, gender
- NHS number, ethnicity, first language
- Address details
- SEND status and details
- EHCP and looked after child status
- Support worker details
- Legal ID - Document type, document number (if provided), and verification status
2. Medical & Health Information
If medical information was requested, this section displays:
- Allergies - With details and severity information
- Medical Conditions - Including any medications
- Dietary Requirements - Special dietary needs
- Emergency Contacts - Up to 2 contacts with phone numbers and relationships
- Healthcare Professionals:
- Doctor/GP - Name, phone, surgery name and address
- Health Visitor - Name and phone
- Social Worker - Name, phone and email (if applicable)
- Consents - Medicine administration and emergency treatment consent status
3. Funding Information
- Funding type and eligibility criteria
- Hours per week and session claim type
- 30-hour code and expiry (if applicable)
- 2-year-old code and expiry (if applicable)
- Other provider details (if accessing funding elsewhere)
4. Parent Information
- Name, email, phone number
- National Insurance number
- Address
- Relationship to child
- Legally responsible status
What to Verify
- Legal ID Document Number - Verify the number matches the document seen
- 30-Hour Code - If claiming 30 hours, verify the eligibility code on your LA portal
- 2-Year-Old Code - If claiming 2YO funded hours, verify the LA eligibility code
- Medical Information - Review any allergies or conditions for safeguarding
- Emergency Contacts - Ensure contact details are complete
- Signature - Check the electronic signature is present
Verifying 30-Hour Codes
For extended (30-hour) claims:
- Note the child's eligibility code from the submission
- Click the link to verify with your local authority's portal
- Confirm the code is valid and not expired
- Check the 'I confirm I have verified the 30-hour code' box
- Optionally upload evidence of verification
- Add any verification notes
Verifying 2-Year-Old Codes
For 2-year-old funded hours claims:
- Note the 2YO eligibility code from the submission
- Verify with your local authority's Provider Portal
- Confirm the code is valid and not expired
- Check the 'I confirm I have verified the 2-year-old code' box
- Optionally upload evidence of verification
- Add any verification notes
Approval or Rejection
- Approve - If everything is correct, approve the submission
- Reject - If there are issues, reject with notes explaining what needs correction. On rejection, the parent must complete all steps again.
Important: Once approved, submissions are included in export files. Ensure accuracy before approving.
