How to enter your child's medical details, allergies, and emergency contacts
Why Medical Information is Required
Your nursery needs accurate medical information to:
- Keep your child safe during their time at nursery
- Respond appropriately in emergencies
- Cater for dietary requirements and allergies
- Administer any required medication
Information You'll Need to Provide
Allergies
- Whether your child has any allergies
- List of all allergies (food, environmental, medication)
- Severity and typical reactions
- Any medication required (e.g., EpiPen)
Medical Conditions
- Whether your child has any medical conditions
- Details of conditions (asthma, diabetes, epilepsy, etc.)
- Any regular medication
- Special care requirements
Dietary Requirements
- Vegetarian, vegan, halal, kosher, etc.
- Food intolerances (different from allergies)
- Any foods to avoid
Emergency Contacts
You must provide at least two emergency contacts:
- Contact 1 (Primary) - Full name, phone number, relationship to child
- Contact 2 (Secondary) - Full name, phone number, relationship to child
Emergency contacts should be people who can collect your child if you're unavailable.
Doctor/GP Details
- Doctor's name
- Surgery name and phone number
- Surgery address
Medical Consents
You'll be asked to provide consent for:
- Medicine Administration - Permission for staff to give prescribed/named medication
- Emergency Treatment - Permission for emergency medical treatment if you cannot be reached
Important: Always keep medical information up to date. If your child develops new allergies or conditions, inform your nursery immediately.
Note: Emergency contacts must be different people from you. They cannot be the same as the parent completing the declaration.
Frequently Asked Questions
What if my child has no allergies or medical conditions?
Simply select 'No' for each section. You still need to provide emergency contacts and GP details.
Can I update medical information after submitting?
Contact your nursery to update medical information. They can request a new declaration or update records directly.
Who can I list as an emergency contact?
Any trusted adult who can collect your child and make decisions in an emergency - grandparents, family friends, neighbours. They must be over 18.
